How to make a referral

Our Referal Criteria and our Out-of-Hours Admissions Criteria have the most up-to-date information about making a referral to the hospice. Alternatively, you can ask a member of staff for more information. We also have information about the Discharge Criteria we work to.

Who can make a referral?

  • Any health and social care professionals (for example GPs, social workers, district nurses) who know the patient can make a referral.
  • Family members can also make a referral.
  • Patients can self-refer too.

How to make a referral

  • If you would like to make a referral, please contact our First Contact Team Monday to Friday between 8.00am and 6.00pm by calling 0300 30 30 400.
  • The First Contact Team will assess your enquiry and refer it to other professionals in the Hospice including the consultant doctors if necessary.
  • They will be able to tell you if an individual or their family is likely to benefit from Hospice support.
  • We always recommend that you contact the patient’s GP before you send a referral to the Hospice.
  • If you are making a self-referral, please be aware that we will contact your GP to discuss whether you will benefit from Hospice care.

Referrers can expect

  • The First Contact Team will make telephone contact with the patient or his/her referrer within 24 hours to assess the urgency of the referral and any immediate problems which need to be addressed.
  • We will notify you of the proposed plan for assessment for community-based care, day hospice and out-patient services within 7-10 working days and for urgent referrals within two working days of receiving the referral.
  • Patients who require admission to St Joseph’s Hospice will be offered a bed within a maximum of three working days.
