Foundations in Palliative Care – Advance Care Planning
This workshop explores Advance Care Planning (ACP) and the skills required to guide patients, families and professionals in the process of decision making at the end of life.
Course Information
Learning Objectives:
- Recognise key national guidance informing the process of Advance Care Planning
- Understand what Advance Care Planning is and differentiate between advance statements of wishes and advance decisions to refuse treatment (ADRT) with regards to establishing preferences of care.
- When healthcare professionals do not engage with Advance Planning in an effective and timely way, understand the implications that this has on patient care outcomes.
- Understand where your own values and beliefs originate and how these can affect patients and families from many diverse backgrounds.
- Identify the ‘signposts’ along a patients illness trajectory when it might be appropriate to initiate an ACP discussion/care plan and the barriers to Advance Care Planning
- Discuss the effective communication skills involved in initiating a discussion regarding Advance Care Planning with a patient and their carer.
