Caring For People In Their Homes
Most People with a serious illness prefer to remain at home if they can. St Joseph’s Hopice Community palliative Care team works alongside other professionals, so that, wherever possible, people needing palliative care, in the City of London, Hackney, Newham and tower hamlets, can be cared for in their own homes.
St Joseph’s Community Team consists of clinical nurse specialists, doctors and social workers. We work with your GP and local services like district nurses to enable you to live comfortably and well at home for as long as possible.
Any individual – the patient, their carer or professional staff – can make referrals to the team. However, it is essential that the permission of the patient's General Practitioner be obtained before our team gets involved and begin caring for the patient.
