Referring A Patient > Who To Refer
The hospice supports individuals with a diagnosis of a progressive and life threatening condition, including those with a non cancer condition, their families, friends and informal carers.
Those most likely to benefit from the service are people who have complex problems or distressing issues arising from such a diagnosis - such that they will benefit from the input of a specialist team.
Common problems addressed at the hospice include:
- poorly managed pain and other symptoms
- psychosocial and spiritual distress in the patient, family members or carers
- the need for respite care on the part of carers
- complex/multiple needs in the final stages of illness
- a need for rehabilitation following debilitative treatment or to manage decreasing function
