Referring A Patient > How To Refer
You can refer a patient by downloading our referral form and either posting it to us at St. Joseph’s Hospice, Mare Street, London E8 4SA or faxing it to 020 8525 6085. The form will be received by our Referral and Admissions Office who may contact you for further information.

If you would like to know more about the progress of any referral you make, please telephone the Referral and Admissions Team on 020 8525 6067.
